Understanding TMD and Jaw Joint Symptoms

The temporomandibular joints (TMJs) connect the lower jaw to the skull and permit the jaw to move when you speak, chew, or yawn. TMD refers to a group of conditions that affect these joints, the muscles involved in chewing, or related structures.

Symptoms can vary. Some patients experience jaw or facial pain, tenderness around the joint, stiffness, difficulty chewing, limited movement, or episodes of jaw locking. Clicking or popping may also occur. However, joint sounds without pain or restricted movement are common and do not necessarily require treatment.

Clenching or grinding the teeth, referred to as bruxism, may contribute to jaw muscle discomfort in some people. TMD can involve different factors, so symptoms should not automatically be attributed to grinding, stress, or the way the teeth come together.

How We Evaluate Jaw Pain and Function

We begin by discussing where you feel discomfort, when it occurs, and whether certain activities make it worse.

During the examination, we assess how your jaw opens and closes and check for tenderness or restricted movement. We may also evaluate the jaw joints, chewing muscles, teeth, and other relevant structures.

Dental imaging may be recommended when additional information is clinically necessary. Digital X-rays and 3-D imaging may be used when appropriate for diagnosis or treatment planning. Because jaw and facial pain can have different causes, the evaluation helps us determine whether your symptoms appear consistent with TMD or whether further assessment is needed.

TMD Treatment Based on Your Symptoms

There is no single treatment approach that is appropriate for every TMD. Recommendations depend on your symptoms, examination findings, and the structures involved.

When appropriate, treatment may begin with conservative measures. These can include temporarily choosing softer foods, reducing habits that aggravate the jaw, and using heat or cold with appropriate jaw exercises.

An intraoral appliance may be considered in selected cases, particularly when clenching or grinding is a concern. Evidence regarding their effectiveness for TMD pain is limited, however, and an appliance should not be designed to permanently change the bite. If symptoms persist or the condition requires care beyond what can be appropriately managed in the dental office, further evaluation or referral may be recommended.

Managing TMD Over Time

Jaw symptoms can change over time, making follow-up useful when treatment or self-care measures have been recommended. During subsequent visits, we can reassess jaw movement, discomfort, muscle tenderness, and other findings identified during the initial evaluation.

Paying attention to activities that aggravate your symptoms can also be helpful. Depending on your condition, you may be advised to reduce persistent clenching, gum chewing, or other habits that place additional strain on the jaw.
